How is the grant recipient chosen?

Personal Story

Applicants are invited to share their stories through a written submission or recording. We value authentic storytelling in any format, including typed text, video, audio, or ASL. This section is designed to highlight your lived experiences, values, and goals and should include a description of your chronic pain, including how it has developed over time; how you hope to use this grant; how the use of this grant will benefit you; the likelihood this will improve your pain and quality of life; and the cost of the treatment, medical equipment, or other needs you require. Applicants choosing the audio or video option will provide a URL for the file, such as a private video on YouTube, Google Drive, etc.

Financial Need

To assess financial need, our grant selection committee considers various factors such as income, household size, care needs, disability-related expenses, and broader economic barriers.

Letter of Medical Support from Provider

If our grant selection committee selects you as a finalist, we will reach out to request a letter of medical support from either the medical provider who diagnosed your chronic pain condition or is responsible for your treatment. This letter should be printed on the medical provider's letterhead and confirm the need for the specific treatment, medical device, or other needs included on this application. Priority will be given to treatments that offer lasting relief or significant life changes and are grounded in evidence-based medicine.

Finalist Interview

To assess your connection to foundation values, clarity of purpose, and lived experience, finalists will participate in an interview. Communication method or style will not be judged. Interviews may be conducted via video (Zoom) or audio (conference call). ASL interpretation is available.
